Families with more than one
(1) junior player will pay only one club contribution. All other players
will contribute a fixed amount towards club running costs.
Payment by cheque is
preferred. Cheques should be made payable to:

No money should be given
without a receipt being issued. Club policy of NO PAY NO
PLAY will be enforced. It is the player’s responsibility to remain
financial at all times.
Players will not be
registered until sign-on fees are paid. This fee is NOT the total fee
for the season.
Non-financial members are
not eligible for club benefits e.g. insurance and voting rights or club
awards.
Birth
Certificates:
Junior players who are
playing for the first time are required to produce a photo-copy of their
Birth Certificate.
Clearances:
Players transferring from
another club, Australia wide, must produce a clearance from their former
club prior to signing on.
If you are transferring to another club from us you will also need a
clearance. To obtain the clearance you must be financial with the club at
the time of leaving.
PLEASE NOTE :
Being financial with your former club includes :
Any monies outstanding paid in full
Any sporting gear whatsoever owned by the club paid
for in full or returned in good condition
Any uniforms whatsoever owned by the club paid for in
full or returned in good condition

Code Of
Conduct For The Coach:
Be reasonable in your demands on young players’ time,
energy and enthusiasm.
Avoid over using the talented players. All players
deserve and need to be given a go.
Remember that children participate for fun and
enjoyment.
Ensure that equipment and facilities meet safety
standards.
Take into consideration the maturity level of
children.
Develop in individuals a respect for the ability of
other players.
Keep yourself informed of sound coaching principals
and the principals of growth and development of children.
Create opportunities to teach appropriate sports
behaviour as well as basic skills.
Code Of
Conduct For The Hockey Player:
Compete by the rules.
Never argue with an umpire. If you disagree, discuss
the matter with the coach later.
Control your temper.
Be a good sport. Applaud the efforts of others.
Treat other players as you would wish to be treated.
Show respect to your coach, umpires, officials, team
members and opponents.
Compete for the “fun of it” and not just to please
parents and coaches.
Avoid using bad language.
Code Of
Conduct For Parents And Spectators:
Encourage players to participate.
Encourage children to participate according to the
rules.
Never ridicule or yell at a child for not meeting
your expectations.
Remember children learn by example.
If you disagree with an official, etc, raise the
matter later with a club committee member.
Support the efforts of all players.
Recognize the value and importance of volunteer
workers.
Remember that children play sport for fun.
Condemn the use of violence in any form, be it by a
spectator, coach, official or player.
Encourage players to follow the rules.
Demonstrate the appropriate social behaviour.
ANY PLAYER SENT OFF FOR
ABUSE OF AN UMPIRE OR ANY OTHER ACTION WHICH WOULD BRING THE CLUBS NAME
INTO DISREPUTE WILL BECOME INELIGIBLE FOR ANY CLUB AWARDS.
DUTY OF CARE
It is club policy that protective equipment be worn
by all players during training and competition.
Protective equipment includes mouth guards and shin
pads.
Injuries sustained from not wearing protective
equipment may void insurance claim eligibility.
All players should bring their own water bottles to
training and games.
VOLUNTEER WORK
It is club policy that Parents/players volunteer to
carry out a minimum of two canteen duties and attend one working bee
during the season and umpiring duty/duties when required in lieu of
being levied a fee towards field rental and maintenance. A
Parents/players commitment to this policy is evidenced by signing the
registration form.

4. Certificate of Indemnity and Release AND
Certificate of Consent to Disclosure - Privacy Act. Both certificates are
on the same page, but there are two pages. One for Adult players and the
other for Parents signing on behalf of Children. Please select the
appropriate form.
This form is to be filled out if
your child has an ongoing medical condition. All Information in this form
will be treated with the strictest of confidence and will Only be used in
an emergency situation.
